John Tyler, the 10th president of the United States, had three first ladies. His wife died shortly after he took office replacing William Henry Harrison who died in office. His daughter-in-law served as first lady for a year or two. Then he remarried and his wife took over.
James Buchanan, the 15th president of the United States, was a perpetual bachelor. His niece acted as first lady during his tenure.
Grover Cleveland, the 22nd and 24th president of the United States had his sister as first lady until he married.
